The analysis

The world's imaging bottleneck isn't machines — it's the people who read them

Medical imaging sits behind the majority of modern diagnoses. Yet the workforce that interprets it is distributed so unevenly that where you are born can change your access to a radiologist by more than two orders of magnitude. This is what that inequality looks like, and why it is getting harder to fix.

A spread unlike any other specialty

The gap between the best- and least-served nations with published figures is roughly eighty-fold. At the top, Sweden, Greece and Italy field more than 23 radiologists for every 100,000 people. At the bottom, Ethiopia manages about 0.3, and much of sub-Saharan Africa and South Asia operates below one — while by credible accounts fourteen African countries have no radiologist at all, which makes the true gap not eighty-fold but unbounded. Few other core specialties vary this violently across borders, which is part of why the number is so rarely stated plainly: there is no single global registry, and the figures that do exist are scattered across surveys, censuses and modelled estimates that rarely agree.

Money helps, but structure decides

It would be convenient if density simply tracked national wealth. It doesn't. Germany, among Europe's richest economies, sits below the EU average — partly because German radiologists don't cover nuclear medicine, so the same clinical work is counted under a different specialty. The United Kingdom, a large high-income country with the world's most rigorous workforce census, trails most of its European peers at around ten per 100,000, with a documented consultant shortfall of roughly a third. How a health system defines the role, trains for it and pays for it shapes the count as much as how much it can afford.

The gap is widening even where things are improving

The most important part of this story is invisible on a static map. In the countries that publish year-on-year data, the workforce is growing — and still losing ground. The UK added radiologists at nearly 5% in a single year while demand for CT and MRI reporting rose 8%, pushing the shortfall wider rather than closing it. Singapore, a genuine success story that has grown its workforce almost 10% a year since 2010, still forecasts dozens of unfilled posts within five years. The bottleneck is not primarily a recruitment failure. It is that imaging demand — driven by ageing populations, screening programmes and ever-more-capable scanners — compounds faster than the years-long pipeline required to train the doctors who read it.

Why it matters, and what to watch

Where imaging capacity is thin, diagnoses arrive late, and outcomes follow. This is why the coming decade's most consequential questions for radiology — how far AI can genuinely extend a scarce workforce, whether teleradiology can redistribute it across borders, how training pipelines are widened without diluting them — all ultimately trace back to this one map. The numbers here should be read as tiers, not decimals: the European figures are broadly comparable, while much of the low-income data is modelled and flagged as such. But the shape of the problem is not in doubt.

On the data. Figures are the best comparable value per country, harmonised to a per-100,000 basis from sources spanning 2016–2025 with differing definitions (headcount vs. full-time-equivalent; diagnostic-only vs. all radiologists). European data draws chiefly on the EU-REST/ESR survey and the RCR census; much of the African and South-Asian data is modelled by the Lancet Oncology Commission and is flagged accordingly in each country's detail. The headline ~80× ratio compares only countries that publish a figure (Sweden ~27 vs Ethiopia ~0.3 per 100,000); because fourteen African countries report no radiologist at all, any finite ratio understates the real gap, and a different choice of endpoint yields a very different multiple. Verify against primary sources before use in clinical, commercial or policy decisions.
